Attached please find the Marriage License Requirements as well as the Marriage Affidavit. 

• Please fill out the affidavit up to line 20. Once you have the affidavit completed, gather the required Original Documentation needed for proof of identity, and visit our office. Both you and your fiancé will need to be present to sign the final affidavit. It will take about 30 to 45 minutes to process the affidavit in the system. 
• The cost for the affidavit is $40.

Please note: The Affidavit will be time-stamped on the final document. You must wait 24 hours from the time stamp before you can solemnize your marriage. The Affidavit is valid for 60 days.
